• Waiver and Release of Liability Form for: Team Prep, Team Prep 2, Volleyball Basics Classes, Basic Team Training and Skills Classes.

• Waiver and Release of Liability Form, Code of Conduct Form and Commitment Letter: All Volley Fest teams will be sent a link to complete paperwork online when invited to join a team.
